The Independent Office for Police Conduct (IOPC) is conducting an investigation following a serious misidentification involving victims of a car accident in Rotherham. The South Yorkshire Police (SYP) mistakenly reported the death of one teenager while declaring another was critically injured, leading to distress for both families.

Initially, police confirmed that Trevor Wynn had died, while his counterpart, Joshua Johnson, was sedated in hospital. The police have since referred themselves to the IOPC after new information emerged that contradicted their earlier announcement. Joshua Johnson’s family described their anguish as “unimaginable,” expressing that they spent significant time at the hospital with Mr. Wynn, believing they were supporting their loved one during a critical time.
